Consent and Assent Forms

Please download the consent and assent form and share with both parent/guardians and young people before the workshop takes place.

Please bring signed forms with you on the day. 

The forms reference photography and media sharing permissions: this is only in the rare case that photos are taken to help promote our workshops. We do not take photographs in most workshop sessions. If there are young people in the group who are not comfortable having their photo taken or shared, we will not take any of your group that day. If you are having any issues accessing the form, or have further questions, please email education@oco.ie
