This report outlines the review undertaken by the Ombudsman for Children\u2019s Office, following the only investigation into State compliance with Children First: National Guidelines for the Protection and Welfare of Children. The investigation was carried out under section 10 of the Ombudsman for Children Act, 2002, which allows the Ombudsman to initiate an own volition investigation.<\/p>\n

Following the investigation, which commenced in November 2008, 11 findings of unsound administration against the Health Services Executive and the Office of the Minister for Children and Youth Affairs. In accordance with the Ombudsman for Children Act, 2002 I made 22 recommendations to improve the system.
